I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Debbie Despina Cartsos / Asteria Belly Dance School (hereafter referred to as instructors).
I hereby agree to release instructors and hold instructors harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgment of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
I will inform Debbie Despina Cartsos or any substitute instructors of any injuries and I will seek out medical attention for any pre-existing or new conditions.
*By signing this I give Debbie D. Cartsos or any other Asteria Belly Dance School instructor permission to use images, videos and sound recordings taken during classes, workshops, performances or other affiliated Asteria Belly Dance events in any media for any lawful purpose which may include, among others, advertising, promotion, and marketing. Images may be combined with other images, text and graphics, cropped, altered and modified. Debbie D. Cartsos is not liable for any further consideration or accounting, and further claims for any reason.
*SAFETY MEASURES:
Dancers are not required to show proof of vaccination or wear a mask. Dancers can wear a mask if they choose so.
*Please monitor your health and refrain from attending class if you're feeling fatigued, have a sore throat or any flu-like symptoms.
**Despina has a zero tolerance policy for harassment, unwanted homophobic, transphobic, racist, sexist, ableist, or ageist behavior in her classes. Despina reserves the right to ask you to leave if you behave in any way that is making an students or dancers feel uncomfortable.
