I am requesting that the Falcon Ridge Charter Scholarship Fund assist in covering the cost of my Pay to Participate Athletic Fee for the following sport. ($45 fee, check one box please)

Cross Country

Requirements & Expectations

1. Athlete must this submit form no later than the second scheduled event of the season.

2. Athlete must maintain academic eligibility throughout the entire season as per the guidelines in the athletic handbook.

3. Athlete must maintain good sportsmanship throughout the entire season.

4. Athlete must maintain good behavior in and out of school.

***Any athlete failing to meet these requirements & expectations will be denied assistance and will be required to pay the fee in full immediately upon request to the athletics office.
