Please fill out this survey if you would be interested in your child receiving the Pfizer Covid-19 vaccine. This vaccine is the only one available for ages 16 and up at this time. We do not yet know when our county will have availability to provide this vaccine at our school, but we anticipate sharing that information with you as soon as we know. Your response will be kept confidential and will only be shared with the health department and/or Kay's Pharmacy.
