Confirmation Sponsor Information Forms 2023/2024 (8th Graders Only)

This form is to gather information regarding your sponsor.

Your sponsor must be a practicing Catholic, over the age of 16, not be a parent, and in a valid Catholic marriage. They must also be committed to helping you on your faith journey in preparation for the Sacrament of Confirmation and beyond.

After submitting this information, your sponsor must complete the a Sponsor Attestation Form to be signed by their pastor. Sponsors who attend St. Luke Parish can submit this form to Max Hall at the parish office. This form can be found in a pdf format on the parish website: https://stlukelakewood.org/confirmation-preparation/. This form should be submitted no later than January of the student's 8th grade year.
