Waiver and release of all claims and assumption of risk. I have read this form carefully and am aware that in registering and participating in this activity/program, I am expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which my minor child might sustain as a result of participating in any and all activities connected with and associated with this activity/program.

I recognize and acknowledge that there are certain risks of physical injury to participants in this activity/program, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child may sustain as a result of said participation. In the event of an emergency, I authorize East Bank Club Staff to secure from any licensed hospital, physician, and/or medical personnel treatment deemed necessary for my minor child's immediate care, and I agree that I will be responsible for payment of any and all medical services required. I further agree to waive and relinquish all claims my minor child may have (or accrue to my child) as a result of participating in this activity/program against East Bank Club, including all of its employees. I do hereby fully release and forever discharge East Bank Club from any and all claims for injuries, damages or loss that my minor child may have or that may accrue to my minor child, arising out of, connecting with, or in any way associated with this activity/program.
