Face Mask Request Form
Hello! If you are requesting a donation of masks from Discount Fabric Warehouse, please complete this form.

We are asking that once you have your donation, please post a photo of your organization with the masks to one of our social platforms or e-mail a photo to fabricdfw@gotfabric.com. We enjoy sharing photos of mask distributions with those who have donated their time to make masks.

There is no limit on how many masks you can request, but requests are fulfilled as masks are donated. It may take a bit of time if you have a large request.
