In order to comply with legislation concerning children's performances we must have a Body of Persons (BOPA) licence. For our application for a BOPA to be accepted this form to be completed.
Failure to complete the form will mean that your child will be unable to take part in performances/concerts.
Please let us know immediately if any of the following information changes after submitting this form (ie. change of address/contact details/medical information) so that we can update our records.

Covid-19 - this form must be completed to ensure we have the necessary contact information for Test and Trace.
