This form is to be completed by a designated Rotary Club RYLA Contact, or other Rotary Club Leadership. Please be sure that your club has budgeted for the following students to be sent to RYLA and that you are prepared to coordinate their transportation to/from the camp.
You will use this form to submit the following:
- Your contact information
- The number of students your club will be sponsoring at RYLA 2024
- The names and gender of those students
Note: After submission of this form, the students along with their parents/guardians fill out the “Student Registration Form”

- Student Application Form (one completed form for each student)
