WWLOA Official's Agreement: By submitting this Official's Agreement Form, I acknowledge and agree that I release and hold WWLOA harmless from any and all claims arising out of any of the activities connected with WWLOA and associated lacrosse activities. I understand I am eligible to officiate at the appropriate level of competition indicated by my experience and rating, and fully understand there are no assurances of the number, type or location of game assignments. I understand that I may decline game assignments. I accept official's fees and reimbursement rates as established by WSLA and WWLOA for the current year. I acknowledge my obligation for payment of dues and fees established by WWLOA and accept the withdrawal of applicable dues and fees from payments rendered in the current year. I understand my registration and association with WWLOA does not create an employment contract or an employment relationship with WWLOA or any lacrosse program. I will act and be considered as an independent contractor and amateur sports official, while serving as a WWLOA official in accordance with Washington statutes. As a WWLOA official, I will conduct myself in a professional manner that demonstrates impartiality, courtesy and fairness to all parties. I consent to receive notices related to WWLOA via electronic transmission as permitted under Section 6.1 of the Bylaws of WWLOA. I will attend an annual rules interpretation session, pass the annual USA Lacrosse Official Rules test, and complete all required annual training. I will choose which games I agree to officiate by selecting my availability and having the ultimate decision in accepting a game assignment. I will apply the Rules of Women’s Lacrosse/Girls Lacrosse consistent with the USAL Women’s Division Official’s Manual and NFHS Rules of Girls Lacrosse (when covering HS games) together with the WSLA defined rules for HS and Youth games. I will abide by the WWLOA Code of Ethical Conduct, as well as the USAL Code of Ethics. I certify that all information provided by me in this form is true, accurate and complete.
