This form for is for pre-registration purposes and to be filled out by the coach of each team.  A minimum of 4 teams are needed to establish a division.  Upon registration deadline (January 20) you will be contacted by the South Jefferson SportsPlex staff to finalize your team if the minimum criteria are met.  You may register more than one team by submitting an additional form at the end.  If you have any questions, please call (315) 203-0179 or email: contact@southjeffsportsplex.com.
