This is the Permission Slip for Campus Life Florida Spring Break 2020.

All registration forms (Permission Slip and Medical Form) and payment for Florida 2020 are due on March 13, 2020.

Pricing:

Early Registration:
Before Midnight on February 14, 2020:              $459

Late Registration*:
After February 14, 2020:                                       $479

* Students will not receive a T-shirt if they register late.

Note: Students will need to bring money for 10 meals on their own during the trip.

The first 120 students with this permission slip, their medical form, AND $50 deposit turned in will receive a $100 matching grant towards the trip.
