******Requests must come from San Diego City College Faculty only******

Please fill out the form below with the understanding that:
- per SDCCD Copyright Guidelines, we cannot copy more than 20% of a book
- We can only accept one request per form
- We can only handle two requests per person per week
- We can only scan books that are already located in City's physical library
- All scans must be stored in a restricted place so that only your students may access it, i.e. Canvas
- If you have requested 20% of a book, and want another 20% from that same book, the scan of the original 20% must be destroyed before requesting the additional scan.
