Participation Form for Easel Art Studio's Art Show in Ravenswood
Thank you for your interest in participating in our art show! We are excited to display your child's artwork! Before dropping off the work, please fill out this form no later than Friday, April 11. Drop off of artwork will begin on Monday, March 31 and last until Friday, April 11. Please make sure to drop off the art at our Ravenswood location (4534 N. Ravenswood Ave.). Also please be sure that your child's name is somewhere on their artwork.

If you have multiple children interested in participating, please fill out this form for each child.

Our art show will be on Saturday, April 12, from 4 to 7 pm in Ravenswood (4534 N. Ravenswood). We look forward to seeing you! Easel's art teachers will be there along with some small bites and drinks!

Pickup of your child's art begins after the art show at 7pm on Saturday, April 12 until Friday, April 18.
