Thank you for your interest in a Multnomah County Adult Care Home Program (ACHP) qualifying test. Complete this form to register for one of the ACHP-required tests on the date listed below.  A confirmation email will be sent after you complete this form.

Qualifying Test Date:  Monday, September 9th, 2019
Sign-in begins at 9:30PM, testing begins by 10:00PM
Location:  209 SW 4th Avenue, Portland OR 97204

Three different tests are offered at this time:
1.  Developmental Disabilities Qualifying Test
2.  Aging & People with Disabilities and Mental Health & Addictions Qualifying Test
3.  Ensuring Quality Care test

Please note: ACHP now partners with Mt. You will only have time to complete one test. If you need to take more than one qualifying test, please select another date to register for additional tests.
