The Healthy Babies Healthy Futures (HBHF) program is about supporting whānau to eat well and move more during pregnancy up until the first 4 years of life.

The criteria to enroll in any HBHF program is;

- You are pregnant or care for a child 4 years old or younger and;
- You live in the Auckland or Waitemata DHB region and;
- You or your child identifies as Māori, Pasifika, South Asian or Asian and;
- You are new to the Healthy Babies Healthy Futures program.  
 
If you meet the above criteria, please read the following privacy statement.

Privacy Statement:

We collect personal information, including your:
• Name
• Mobile number or email
• Pregnancy status
• Number of children under 4 in your care
• DHB location
• Ethnicity
• Current lifestyle
 
We collect personal information in order to support people's health goals, aspirations, and expectations of how we deliver programs. We need this information to fulfill our contractual obligations with the Ministry of Health of New Zealand to keep this public health program accessible.
 
If you choose not to complete this enrollment form, we will not be able to teach you.

Once you complete and submit this form. Your information is kept safe in a password-protected shared google drive. Your enrolment form is accessible to HBHF contracted staff only. You will be contacted to confirm your details and discuss a starting date to learn with us.

The HBHF coordinators are:

Your personal information will never be shared with any external services other than the ones mentioned above.

You have the right to ask for a copy of any personal information we hold about you and to ask for it to be corrected if you think it is wrong.
