Dear Neighbor,

The Prospect Heights Neighborhood Development Council (PHNDC) in conjunction with the New York City Department of Transportation operates the Open Streets program on Underhill Avenue. As the City has announced the program will become permanent, PHNDC is interested in how the Open Street has affected your life as a resident of Underhill Avenue.

There are five short sections of questions:

1) Information about you and where you live
2) Your overall impressions of the Underhill Avenue Open Street
3) Your opinion on how well we are doing operating the Open Street safely
4) Your feedback on programming and events
5) Your experience of the potential negative impacts of living on the Open Street

Your responses will be used to inform operating the Open Street for the 2021 season, and the long-term planning beyond 2021.
