Religious School Enrollment Agreement
Dear Parents,
Thanks for taking the time to register your child for the Forest Hills Jewish Center Religious School.  There are TWO PARTS to registration.

1. Complete the ONLINE Enrollment Agreement below

2.  Print out and complete the HARD COPY Tuition Payment Contract which you can find here and begin your tuition payment. https://docs.google.com/document/d/12FIVEqx0dZyDJQCRV0XfkBmvTZVtMGsyTMIkFhSchrY/edit?usp=sharing

The tuition can be paid by check--dropped off or mailed to Laurie Worthman, the office manager at FHJC; or by credit card over the phone with Laurie at 718-263-7000x203.

 You will receive an automatic email acknowledging completion once you have filled out all necessary parts of the Enrollment Agreement.  If you do not receive the acknowledgment, please return to this form to edit or complete it as needed.
