Event Request Form
Filling out this form will enable the Event owner to organize and coordinate all the event details and resources needed for the event ensuring that all support departments are notified and prepared for the event. It is recommended to first check the ASFM calendar and the desired venue’s calendar to see the viability of the event before submitting the form.

For your reference, please consult the Events Policy and Procedures Manual. Please note that the form is intended for events happening inside the school premises, and that it does not support multi venue nor multi day events, as of now.

Type of Event *
An event at ASFM is defined as a planned occurrence that is of significance to its targeted audience, and has a pre-arranged sequence of actions which would require coordination and support from at least two of our service departments, or support of one service department and more than 20 people attending. Please note that an event of more than 300 external visitors requires the presence of an ambulance with paramedics throughout the whole duration of the event.

Owner of the Event *
This is an ASFM person who is organizing / requesting the event and is responsible for it. Please indicate the ASFM email address of the person who owns the event (@asfm.edu.mx).
Section of the Owner *
Indicate the section of the school from where the owner is from.
Authorizer of the Event *
An authorizer is an ASFM administrator / director / principal.
