Summer Story Hour Registration
Story Hour at the S.W. Smith Memorial Public Library is for children ages 0-6 with their adults. Children who have attended Kindergarten, but have not started First Grade are welcome to attend Story Hour. If your child completed First Grade, please fill out the SummerQuest Registration program form. Please fill out one registration form per child. Story Hour will run weekly from June 12-July 17, 2024 at 10am on Wednesdays. Story Hour will have stations for reading, craft, and play. Community partners will attend some Story Hour sessions to present a fun learning station relative to their mission. All Story Hour programs will be held at 201 E Maple Street in Port Allegany unless otherwise stated. Registration ends June 1, 2024.

Story Hour is funded by a grant from the United Way of Bradford Area Inc, Port Allegany Campaign. The grant funds are dispersed every April. There are limited funds to run Story Hour. The library typically hosts twenty Story Hour meetings per year on a $1,000 budget. Each session of Story Hour, Summer, Fall, and Winter, has it's own set budget. Summer is the most popular Story Hour session with an average of 60 registered children and their adults in attendance. Due to such a high attendance number, the library asks for registration before the program begins to make sure the library has enough in the Summer Story Hour budget to accommodate all of the registered children. The library understands that sometimes attendees want to bring unregistered children to Summer Story Hour--they are older siblings, visiting friends or relatives, a playdate, etc. While the library is happy to welcome everyone, the  Story Hour budget is limited and hosting children who are not registered impact that budget. Case in point, the library had so many unregistered children participate in Summer Story Hour 2023 that the budget for Fall 2023 and Winter 2024 Story Hour needed to be used as additional funds. Fall and Winter Story Hour went well, but without a budget they did not offer children's performers, new toys/games, and craft supplies were so low that projects were pretty basic (glue, paper, etc). Children's performers at Story Hour are also given an attendance number from the registration form collection. If they are told 52 children are registered and attending, they are only going to bring enough supplies for 52 children. The same goes for program supplies: if the library has 52 children regististered, the library is going to have enough program supplies ready for 52 children. In Summer 2023 we did not have enough supplies to accommodate some children and that was very upsetting to children, their adults, and the library staff. The library's love of children's programing is unlimited, but the budget isn't. Please help us create a successful program by registering each child you are bringing to Story Hour, even if they are coming for just for one day. If older siblings are accompanying you to Story Hour, please explain to them that the Story Hour program is for the younger child(ren) and while they are more than welcome to hang out in the library too, there may not be enough supplies for them to participate in the Story Hour program. Children over age 6 who have completed Kindergarten are welcome to attend SummerQuest and should register for that program. Thank you for your consideration and cooperation.  *

If you do not have a library account available, please write "contact me about applying for a library account." If there isn't a current library account, one must be opened before Story Hour begins. Please bring proof of address to the library to open your library account.  *
Photo Release Form: The undersigned agrees to give permission to the S.W. Smith Memorial Public Library to use photographs of my child and my child's adults for the purpose of publicizing the library in either general library promotions and/or in publications that may act as fundraising ventures. The photo will most likely not contain a caption identifying any individuals, although one may occasionally accompany the photo. *
Allergy information: occasionally Story Hour involves snacks, beverages, and outdoor play. If you child has an allergy to food, insect bites or stings, animals or dietary restrictions, please write them here. We ask that the adult in attendance read ingredient labels  before food or beverages are given to their child. The library will do its best to provide an alternative food or beverage to a child with a known allergy or sensitivity to a listed ingredient. We encourage the adult in attendance with the child to come prepared to address known allergic reactions with any medication the child may need if an allergic reaction occurs. If you child has no known allergies, please write "no allergies." *
Story Hour may involve outdoor play. Please take precautions to protect your child from summer sun and temperatures and have them wear outdoor play appropriate footwear. If your child is walking, your child needs to wear footwear inside the library as well. The library does block off outdoor play areas to keep traffic away, but please be aware of your child's location during outside play for their safety. *
