(1 of 7) I will bring no more than 2 dogs and agree to pay the non-refundable pet fee of $45 per dog per night. My credit card may be subject to a pre-authorization of up to $450 security deposit upon check-in. *
(2 of 7) I declare my dog(s) are fully housebroken, over 1 year old, healthy, disease-free, non-aggressive, and free of pests. My dog(s) are fully up to date on vaccinations. *
(3 of 7) I understand that my bungalow is subject to management inspection at any time during my stay (for emergency purposes, excessive barking, anything that management deems necessary for safety reasons) and prior to my departure to assess any damages. I also agree to make my bungalow available to housekeeping and/or maintenance needs during my stay and will arrange to have my dog(s) out of the bungalow to accommodate this service. *
(4 of 7) My dog(s) will not be left unattended in the bungalow without being in a crate (kenneled), and I will not allow my dog(s) to be on the beds, furniture, nor will I bathe my dog(s) in the shower (bike wash station is available for dog baths). I will not leave my dog(s) unattended in my vehicle or elsewhere on the property.   *
(5 of 7) My dog(s) will be on a controlled leash at all times when outdoors and I will not take my dog(s) near other buildings or in the private meadow.  I will clean up after my dog(s), disposing of waste in designated receptacles only. *
(6 of 7) I understand that barking, lunging, or other behavior that disturbs or interferes with other guests’ enjoyment or interferes with staff’s ability to do their job will require the removal of my dog(s) from this property and if this requires me to terminate my reservation early, I understand I will not be refunded for any unused portion of my stay. *
(7 of 7) Damages caused by my dog(s) to the room, its furnishings, or any other part of Moab Springs Ranch property and grounds are my sole responsibility. I understand that my security deposit will be processed to cover the cost of repairs, replacements, and services to return my bungalow to it’s original rental condition. If damages exceed the deposit, I understand my credit card will be charged. *
