Student Research Survey Submission Form

If you are a Longwood University student and would like to post a survey for a class or independent research project, please complete the following form.

Remember that you will likely need to obtain approval from Longwood's Institutional Review Board (IRB) before you administer your survey. Please visit the Longwood IRB Committee webpage for more information. 


You should receive an email once you complete this form. This email serves as notification of successful submission of your survey. Please use your live.longwood.edu email address when completing this form.

Your Student Research Survey will be posted on the Longwood University Student Research Survey site within one week of completing this form.

***IMPORTANT***- At the end of your survey, in the "thank you for your participation" box that appears after the subject submits the survey, please include a link to a second survey (link provided below) to record participation. You should provide assurance that clicking on the link and entering their contact information will not be connected with their individual responses to your survey (only their participation in the survey overall).  The second survey is not connected to the individual responses on your main survey and will only be used to record participation and determine winners for the end of the semester prize drawings.
