This Google form replaces the paper fee form previously used for assessing fees and is REQUIRED to be completed by all families.
This form is being used only to ascertain the appropriate fees for each student. Actual fees billing will be processed through the FACTS system and parents will be notified via email from the system that a fees bill has been posted to their account. Parents may choose to pay fees via check or through the FACTS system. Check payments are to be made out to Regis Middle School and sent to the Main Office.
Fees are the responsibility of the parent/guardian and are expected to be paid in full. All tuition and fees accounts are to be cleared to $0.00 by June 1, 2022.
