To ensure the safety of players and volunteers during our continued navigation of COVID-19 protocols, we are asking players to complete the following contact tracing form.  This will help make the check-in process more efficient for players attending conditioning ice times.

Arrival is a maximum of 30 minutes prior to the scheduled ice time.

**WE ASK THAT WHERE POSSIBLE PLEASE HAVE YOUR CHILD FULLY DRESSED WITH SKATE GUARDS ON TO LIMIT THE NUMBER OF PARENTS IN DRESSING ROOM AREAS.**
