It's time again for the PTA to publish the student directory for the 20-21 school year.  You do not have to be a PTA member to be included in the directory.  Our goal is for 100% of families to respond to this form whether you wish to have your information published or not.  Directories are distributed to PTA members as part of the membership fee.  Additionally, anyone can purchase a copy or additional copies of the directory at https://maplegrove.new.memberhub.store/store.  We cannot receive any contact information from Jeffco Connect, so all information obtained for the paper directory will come from this form.  In addition, joining the PTA doesn't automatically mean we have your contact information and consent for the directory.  We still need you to complete this form.   If you have any questions, please contact Liz Grevengoed at egrevengoed@gmail.com
