This application should be used to apply to the Great Oaks' Project SEARCH programs at Cincinnati Children's Hospital Medical Center, Fifth Third Bank Operations Center and Xavier University.  Please complete this form in its entirety.

* Final placement into this program will depend on the Individual Education Plan (IEP) team meeting.

* A two-week trial period will be required of all accepted students.

* Student records concerning the applicant may be transferred to Great Oaks from his/ her affiliated school for review by professional staff.
