Dear Parent(s),

The following information is for students interested in trying out for a St. Agnes Cathedral School 7th & 8th Grade Fall Athletic Team.

In addition to completing this online form, the RVC NYSED Health History for Athletics form (will be sent home with your child), the Medical Release (below) and an up to date Physical Form must be submitted prior to tryouts. Please check with Mrs. Keating or Ms. Poore to see if your physical form is up to date.  The physical exam can be conducted by a Doctor of your choice.

The Medical Release Form can be completed below. Please complete all information in its entirety in order to participate in tryouts.

All forms and the completed online form are due by Tuesday, September 13th!! Please note, we are unable to take submissions after the 13th. 

There is a $150.00 non-refundable fee due once the team is selected.  

Students are required to attend all practices.  If your child is in school, they will be expected to be at practice.  If they are not in school, they will not be permitted to attend practice or be at the game that day.

Parents, not the coaches, are responsible for transporting their children.  The students are expected to be on time.

The team is not to interfere with the academic life of the student. Team members whose grades are borderline or failing will not be allowed to continue on the team.  

It is expected that all student-athletes will behave in accordance with the school discipline code as noted in the Parent-Student Handbook.
