Complete this form and return ASAP. ONLY A RETURNING COACH CAN REQUEST TEAM COLORS. Returning teams need to fill out a form to keep their previous color. Remember there cannot be duplicate colors within the same age group. Existing colors are only available if a return team gives up their prior color.
