Information collected on this form is used to set your student up in our Student Information System.  Please fill out information accurately and to the best of your knowledge. One form per student.

After submitting this general information, you will also need to complete the Annual Online Registration Form and fulfill other enrollment requirements. A District Registration Team Member will contact you with the steps to completing registration and enrollment.
