Please complete this form for EACH DAY that your child is absent from school.  This form must be completed from a parent/guardian email NOT the student's email.  Upon return to school, your child needs a written note signed by a parent/guardian or a doctor's note for each absence.   They need to get it signed by each teacher and drop off signed note in room 119 at the end of the day.
