You have been asked to complete this survey following your recent experience of accessing services at Sexual Health Sheffield. By providing your feedback you will help us in continuing to improve and ensure our service is easily accessible and meets the needs of all our service users.

Completion of this survey is voluntary and it will not affect the care you receive from our service or any part of the NHS if you choose not to take part.

Completing the questionnaire

• Your answers will be given anonymously and will be treated in complete confidence.
• Please answer these questions about your overall experiences of the service, including your most recent visit.
• Please answer each question as fully as possible - there will be a combination of score based and free text questions.  If there are any questions you would prefer not to answer, or do not wish to explain your answer you will not be asked to give a reason.
• Please compete and submit the survey as soon as possible after your appointment in clinic so that your recollection of your visit is more immediate.
