I have agreed to be interviewed at the above date, time and place. I understand that I will be recorded. I hereby authorize The Center for Puerto Rican Studies, Hunter College (CUNY) and those acting pursuant to its authority (collectively, “CENTRO”) to:
Photograph, videotape, audiotape, transcribe or otherwise record, in any medium, my participation in this Interview;

Use, modify, reproduce, public, exhibit and/or distribute any and all such recordings, in whole or in part, in any manner or medium now know or hereafter developed (including without limitation, the classroom, print publications, webcasts, podcasts, television, and websites), an unlimited number of times in perpetuity throughout the world, for any purpose that CENTRO may deem appropriate, including without limitation educational, promotional and commercial uses; and

Use or license others to use my name, image and biographical material in connection with any such recordings or uses, but not as endorsement of any product or service.
I hereby waive the right to inspect or approve any such recordings and uses. I understand that CENTRO will be the owner of all rights in and to such recordings and uses, subject to the restrictions described in this consent and release, and retains the right to use the recordings for any purposes.
I hereby release and hold harmless CENTRO from liability for all claims that may arise by me or others in connection to claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, copyright, or any other legal matters with regards to CENTRO’s activities as authorized by this consent and release.
I am age 18 or older, or if I am under the age 18, my parents or legal guardian will review this form and act on my behalf.
