I, parent/guardian of the registrant, a minor, agree that the registration and I will abide by the rules of US Soccer, US Club Soccer its affiliated organizations and sponsors, Boston Bolts and their affiliated organizations, facilities and sponsors.  Recognizing the possibility of physical injury associated with soccer and in consideration for US Club Soccer and Boston Bolts accepting the registration for its soccer programs and activities (the "Programs"), I hereby release, discharge, and/or otherwise indemnify the US Club Soccer and Boston Bolts and its affiliated organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities utilized for the Programs, against any claim by or on the behalf of the registrant as a result of the registrant's participation in the Programs and/or being transported to or from the same, which transportation I do authorize.  I do hereby certify that my child is in good health, has no COVID symptoms like fever, cough, etc.
