The school will not give medication to your child unless you complete and sign this form, and the Principal has agreed that staff can administer this medicine.

This form is to be completed by the parent/guardian.  It enables the school to ensure the correct information has been received from parents and to monitor and correctly support the use of medication in school.  If a pupil requires several items of medication in school the appropriate details should be provided.  On receipt of Form AM2, Ashgrove PS will complete the "Agreement of Principal" form.
