本申込における，個人情報の取り扱いについて，以下のように定めます。

(1) 個人情報の利用目的

・申込情報を確認し，適切な情報を提供するため

・当日の運営に係る事務または業務を遂行するため

・主催者、共催者からの広報活動への利用のため

(2) 保有する個人情報の提供

・本調査で管理する個人情報は，次に掲げる場合を除いて主催者、共催者以外の第三者に提供することはありません。

・本人の同意があるとき，又は本人に提供するとき

(3) 個人情報の開示，訂正等について

本調査の担当事務は，利用者本人からの自己情報の開示，訂正の求めがあった場合は確実に対応します。

Handling of Personal Information

The handling of personal information in this application is stipulated as follows
(1) Purpose of use of personal information
　To confirm the application information and provide appropriate information.
　To perform administrative work related to the operation of the event.
　To conduct administrative work related to the operation of the event and to provide appropriate information.

(2) Provision of personal information
　Personal information managed by this survey will not be provided to third parties other than the organizers and co-organizers, except in the following cases

　When the individual concerned has given consent, or when the information is provided to the individual concerned.

(3) Disclosure, correction, etc. of personal information
　The administration in charge of this survey will respond to requests for disclosure or correction of personal information from the user himself/herself without fail.

(4) Inquiries about personal information handled in this survey
