This form is for your school to track Lateral Flow Device (LFD) test results.
It is important to record results, so the school has a record of when a staff member has last tested and the result of that test.

What actions should you take after a test?

First, log your test result at: https://www.gov.uk/report-covid19-result
Then, complete this form.

If NEGATIVE, no further action is necessary after completing this form.
If VOID, please retest immediately.
If VOID for a second time, complete the online form, self-isolate with your family and book a PCR test to confirm the result.
If POSITIVE, complete the online form, inform Mitch immediately, self-isolate with your family and book a PCR test to confirm the result.
