CSC Birthday Sign Up
We would love to help provide a gift for your child(ren)'s birthday. Please submit this form at least ONE MONTH in advance of their birthday. Forms submitted less than one month in advance are not guaranteed to be filled before their birthday. We will notify you when your child's gift and cookie are ready for pick-up, but you can expect it to be ready on the Wednesday before his or her birthday!
We are asking donors to spend $25 maximum on birthday gifts. Please keep this number in mind when filling out this form. Thank you!
