I, the undersigned parent / guardian of the student(s) whose name(s) appear in this form, hereby apply to enroll this student in VERNAL THEATRE: LIVE'S BROADWAY BABIES CLASS. In so doing, we acknowledge that we have read and agree to the guidelines as stated below.

1) Drop-off Time/Attendance: Theatre doors open at 8:45am OR 12:45pm. I agree to SIGN IN MY CHILD IN PERSON. Classes begin promptly at 9:00am OR 1:00pm.

2) Pick-up Time: Classes end at 12:00pm OR 4:00pm. I agree to SIGN MY CHILD OUT WITH THE TEACHER'S AIDE IN THE LOBBY. After 15 minutes, the staff will begin calling parents and a $20/hr child care fee will apply payable in cash upon pick-up of any student.

3) Water: The theatre does not have a drinking fountain. Students are permitted to bring a water bottle with WATER in it and his/her name on his/her water bottle.

4) Dress code: Loose clothing that allows for movement in best. Shirts should be along enough to tuck in and midriffs and spandex shorts (unless under regular shorts) are not allowed. All students must bring or wear some type of closed-toed, soft-soled shoes (such as gym/tennis shoes) with socks.

5) Medication: It is understood that students bringing any type of non-prescription medication must first submit a note to the instructor by a parent indicating the frequency that the medication should be taken. We agree that students must ultimately be responsible for knowing when and how much medication to take and that the theatre is not responsible for administering proper doses at proper times to students. (The same is applicable for asthma inhalers).

6) Conduct: INAPPROPRIATE BEHAVIOR AND/OR PROFANITY WILL NOT BE TOLERATED AND STUDENTS WILL BE DISMISSED FROM THE THEATRE FOR THE REST OF THE APPLICABLE TERM(S) IF BEHAVIOR BECOMES AN ISSUE. It is understood that if a student violates theatre policy, the parent will be contacted in a timely fashion and steps will be taken to correct the matter. We agree that if the student’s behavior does not improve, the theatre will dismiss the student and that in all cases, the theatre has final say in any dismissal. It is understood that if at any time, a parent can no longer support Vernal Theatre: LIVE policies, they are encouraged to withdraw their child. It is understood and we agree that parents and guardians should not upbraid, insult, or abuse any member of the school administration, faculty, parent, student or staff on or off theatre premises.

7) Refund: It is understood and we agree that no refund or reduction of tuition will be made for the withdrawal of a student after the start of the class or in the event the student is dismissed due to misconduct or any other physical or behavioral problems. In the event of accident or illness, the theatre and parent, at the theatre’s sole discretion, may share in the unused tuition 50% each.

8) Disclosure: It is understood that the parents/guardians of students enrolling in Broadway Babies, are obliged to inform the theatre at the time of application, of any physical or behavioral problems the child has which have required professional treatment; it is understood that the theatre may decline to accept such child, or may revoke such acceptance, if it is apparent the child may not have a successful experience in the class or may cause other students to not have a successful experience.

9) Liability: We hereby release and agree to indemnify and hold harmless Vernal Theatre: LIVE and all its officers, directors, employees, agents, and representatives whatsoever from any and all losses, claim, damages, liabilities, cost and expense (including, but not limited to attorney’s fees) which they, or any of them, or student may sustain or incur in any way arising out of or in conjunction with this student’s participation in any theatre activities. It is also understood that such conditions also relate to acts of nature.

10) Lost/stolen/damaged property: We agree that Vernal Theatre: LIVE is not responsible for the loss or damage to a student’s personal belongings for any reason, and that the theatre strongly recommends that students not bring items of value to the theatre. Any damage to theatre property will be repaid by the student starting with a minimum repayment of $25 up to full value of the damaged item(s).

11) I hereby authorize Vernal Theatre: LIVE, without restrictions of any kind, to use any photographic image or voice, singly or in combination, in a video, photographic image, digital image, or series of such productions for use by Vernal Theatre: LIVE. I agree that no monetary compensation is implied in or expected from this release. I represent and warrant that I am the parent or guardian of the minor(s) whose name(s) appear(s) above. I hereby release Vernal Theatre: LIVE as set forth above from any claims and causes of action that I may have against it from my child(ren)'s participation in a Vernal Theatre: LIVE program. I hereby fully and unconditionally guarantee my child(ren)'s releases as set forth above.
