Presidents' Day Cup Coaches Check-in
Thanks for completing the electronic check-in form.  Per COVID policies, each team is permitted a maximum of TWO named adults (Coaches/Chaperones) in attendance and up to 12 rostered players.  The below survey will collect each of the following:

1. Club and Team Name
2. Name, email and phone number of head coach
3. Name of the additional permitted adult (IDs for both will be checked at the door)
4. Submission of hotel rooming information
5. Certification of accurate team roster

Convention Centers are now requiring showing proof of impact in order to secure use of their facilities in future years and this information helps us keep the cost of participating down for you all for future years.  Your cooperation is greatly appreciated!  Please provide the name of the hotel your team is staying at, along with the last names of all coaches/players staying each Friday and Saturday evenings.  This will help us best account for the impact of the event as described above.
