BEST Session 2 Afternoon Registration 21-22
This form is for any parent/guardian that wishes to enroll their student in an after school BEST Program for our Second block of BEST programming starting November 29th, 2021 and ending on January 21, 2022 . Please submit ONE form PER student you wish to enroll in an after school BEST program.

Enrollment will be on a first come, first serve basis. If your chosen program fills, your student will be "waitlisted", or you will have to opportunity to choose another program if spots are available. If a spot opens up we will pull additional students, in order, from the wait list to fill the open spot(s).

Programs are listed as "Title of Program (Staff Member; Day it runs; Grade Levels that can enroll)"  PLEASE select only one program per day.
