If you would like to submit a mask exemption for your son or daughter please complete this form, in its entirety.  If you have any questions, please contact your department head.  You must submit a separate form for EACH child in your family, for whom you are submitting an exemption.  Exemptions for children under the age of 4 are not necessary.
