Requirements for Students in extracurricular activities:
1.  Pay sports participation fee prior to the first game of the season.  Middle School / High School Fee $150, jersey fee $25

2.  Prior to the first practice, 9th grade students must have a Health History and Physical Exam completed. Use the form included in the August Newsletter.  

3.  Demonstrate academic responsibility by having no more than 20% of your formative assignments missing for two consecutive weeks throughout the sport season, and maintain passing grades of 2/C or higher in all classes. And memorize current school memory verse.

4.  Student athletes are required to participate in all athletic fundraisers in support of the CMPA athletic program. Parental support is needed during each season.

My typed signature below acknowledges that I have read and understand the requirements for my student athlete to participate in athletics.  I understand that my athlete will not be allowed to play in the games until the fees have been paid, or payment arrangements have been made with Athletic Director, Ryan Beard.  I also understand that my 9th grade student athlete will not be eligible to practice unless the physical exam paperwork has been completed and turned in to Mr. Beard. Notice of Risk for Student Athletes
We realize that such activity involves the potential for injury that is inherent in all sports.  We acknowledge that even with the best coaching, use of the most advanced protective equipment, and strict observance of rules, injuries are still a possibility.  On rare occasions these injuries can be severe as to result in total disability, paralysis or even fatal.  We, the typed undersigned, understand the dangers of practicing, playing and participating in sports.  We specifically acknowledge that we have carefully read and understand this Notice of Risk for Student Athletes and will not hold Calvary Meridian Prep Academy responsible for any unforeseen injury.

Code of Conduct:
All competitions must be conducted with a high standard of courtesy, fair play and sportsmanship. All of those involved share responsibility.  
Athletes: Athletes must demonstrate qualities of courtesy, good sportsmanship, and Christ-like attitude.  Spectators:  The spectators, both students and adults, need to demonstrate courtesy, good sportsmanship, and Christ-like attitude by cheering in a positive and appropriate manner for their team and never against the opponents.  This is evidenced by the absence of booing or loud negative outbursts to either team or referee.  Spectators must show proper acceptance of official's judgement and the coach's decisions.  Anyone not observing this Code of Conduct will be asked to leave and must contact the Athletic Director and Principal before returning to any other school events.   Parents are responsible for the transportation to and from games; parents may choose to arrange carpools on their own accord, but athletes are not allowed to transport other student athletes to away games.  The coaches must have written notice of who is allowed to transport the student other than the student's own parent.
