Under 18s Parental Form

OPM Hockey club prides itself on safeguarding children.  This form is to enable every parent of under 18s in the hockey club to agree and be made aware of our club policies in relation to children.  

As a parent we want you to feel confident that your child/ren are safe whilst at our hockey club and to enable us to do this please complete the following form for your child.

Any concerns or problems that arise from this form that you are not happy with, please contact the club welfare officer (details can be found on our website)

Please tick the following boxes that you as the parent agree with: (Any that are not ticked will be discussed with yourself, the team captain and welfare officer to come up with a solution to the matter)

Responses will be kept confidentially by the OPM Hockey Secretary for 1 year and then deleted.

As a parent do you agree to your child travelling in vehicles with other players.  The club will try to ensure there is always more than one adult with your child on this journey to help safeguard them. *

As a parent do you agree to photographs or filming of your child in games or training situations for use of the club for coaching/training purposes. *

As a parent the club would like you to agree to your child using the changing facilities.  These facilities are solely used by the coach, team and officials, unless public facilities are the only provision made by the home team.  Under aged players are under no obligation to attend the changing rooms at any point in order to participate in training, or on match days. *

As a parent you agree to your child being added to the teams WhatsApp/Facebook group if they have access to these forums and wish to be part of it. The club takes no responsibility for the content in these groups but wish parents to be aware they exist and children are allowed to participate in the groups. *

As a parent, you and your child will read the code of conduct and club constitution and will abide by the rules of the club. *

As a parent, you will raise any safeguarding concerns/problems immediately to the club welfare officer, this will enable the club to safeguard your child but also protect others. *

Please contact the welfare officer or club captain if you wish to discuss any parts of this form you are not happy to agree to and arrangements can be made for your child.  By returning this form you are agreeing to the above points and are happy for your child to be part of our hockey club. *
