PARENT/STUDENT AGREEMENT
I have received and read the Student Handbook that describes the Maywood Public
School District’s discipline policies, regulations, rules, and expectations to
be followed by students enrolled in Maywood Public Schools, including the Drug
Free School Policy. My child and I have discussed these policies and understand that we must comply with them. The undersigned, as student, agrees to follow such conduct and discipline rules. This receipt also serves to acknowledge understanding that the district’s policies of non-discrimination and equity exist in the handbook, and that specific complaint and grievance procedures exist therein which should be followed and used to response to any complaints of harassment of discrimination. The Parent-Student Handbook can be viewed online at www.maywoodtigers.org.
This signed receipt further acknowledges receipt of the district and personal technology policies in the Parent-Student Handbook. I give permission for the student to access network computer services and accept responsibility for such use and any consequences and liability, and the student agrees to fully comply with such policy.
