Before submitting this form, please be sure that you have followed these steps to create your PARENTVUE account:
1. Open your browser and navigate to https://parentvue.cusdk8.org. Select "I am a parent.”
2. Click more options, click activate account, accept privacy statement.
3. Sign in with the activation key (provided in an email from the district) and follow the prompts.

To log in to your PARENTVUE account:
1. Open your browser and navigate to https://parentvue.cusdk8.org. Select "I am a parent.”
2. Sign in using the email address and password that you selected when you activated your acount.
