As an agency which plays a pivotal role in addressing the negative impact that inequities and overall lack of inclusivity have on our communities, we care deeply about your experience with us. That’s why we’ve created a platform for individuals who engage with Prevention Links to submit feedback, report harms and share experiences for us to address.

If you would like to give anonymous feedback, please fill out and submit this anonymous Prevention Links Community Concerns Form. The Diversity, Equity & Inclusion (DEI) Committee will review all concerns submitted on a monthly basis and address all concerns accordingly.

Please note that by filling out this anonymous form, Prevention Links will not have the ability to provide direct feedback or ask questions regarding the concerns, harms, feedback and/or experiences you submit.
