I understand that my child will be under the supervision of leaders from Shellharbour City Anglican Church. All leaders have completed Safe Ministry training.

I give permission for my child to travel in leaders’ cars during the weekend if necessary.

I authorise the youth group leader to arrange for my child to receive medical treatment when they deem necessary if I am unable to be contacted.

I understand there may be photographs or video footage of my child taken during this camp to promote Crosslife

I will make full payment online by direct debit by Friday, 7th April or make alternate arrangements by contacting Murray.

I will need to pick my child up from camp if their behaviour is offensive or dangerous to others (no refund will be received).
