Positive COVID Reporting 2022-2023
If your child tests positive for COVID-19, please complete the following form. 

In addition to submitting the form below, students who test positive for COVID must follow a 5-10 day quarantine: 5 days at home Day 6-10 may return to school FULLY MASKED if fever-free for 24 hours with no fever-reducing medication Cafeteria eating plan (table on stage) followed for remainder of 10 day timeline.  
If a parent prefers that their student not utilize the stage covid-impacted table, your child needs to remain at home through the 10 day plan.

For students who have been exposed to someone who has COVID but has not tested positive for COVID, we recommend home daily testing for 7-10 days.  This student may attend school provided that they are not symptomatic. We recommend that students who have been exposed to remain masked in school.  Students may NOT come to school if symptomatic. Isolated eating is not provided by the school for students who have been exposed.
