Eligible Bus Riders
Transportation registration is required for each Summer School session even if your student has been registered for transportation in previous years.
In accordance with Board Policy EEA, elementary students (grades K-5) who live ore than one mile from school are eligible for school bus transportation. Secondary students (grades 6-12) who live more than one and one-half miles from school are eligible for school bus transportation.
Bus Stop Assignment
Each eligible bus rider may be assigned a school bus stop for both pick-up and drop off. The two stops could be at different locations.
Alternate Address for Pick-up/Drop off (daycare, relative, work, etc.)
Your student may be served at an alternate pick-up and/or drop off address if the alternate address is in an area eligible for school bus transportation. This is determined by the Board Policy listed above and additional department policies. Please submit this registration form including the alternate address and a Transportation Department staff member will contact you regarding your request.
Rules for Riding the School Bus
Riding the school bus is a privilege. Riders must follow all the rules described in district policy EEACC, "Student Conduct on School Buses". This policy is available from your student's school or online at: http://policy.osba.org/creswell/E/EEACC%20D1.PDF
Special Note for Parents of Kindergarten Students
All Kindergarten students must have an authorized adult present at their assigned stop. Please specify in the area provided in this form, the names of any adults Creswell Transportation is authorized to release your student to. If an authorized adult is not present, your Kindergarten student will be returned to Creslane Elementary and you will be notified.
Special Note for Parents of 1st through 5th Grade Students
Please specify whether your student may be dropped at their assigned stop without an authorized adult present. In the case that an adult is not present and you have specified that an adult MUST be, your student will be returned to Creslane Elementary and you will be notified. A space is provided on this form for you to list any adult you authorize Creswell Transportation to release your student to.
