I understand that SUMMER requirements include either BOTH sessions OR one camp and one session (prefer Session 2). CT dancers will be allowed to make up missed summer classes across both sessions. Initials: *

I agree to pay the $185 CT cost on August 1, 2019. This includes $75 (rehearsal fee), $50 (costume fee; $20 extra if performing 2 dances in Act 1), and $60 deposit (refunded on December 1st with only one absence). The only refundable portion is the deposit IF only one rehearsal is missed. Initials: *
