Family Assistance Program: Welcome Package Application
𝐀𝐛𝐨𝐮𝐭 𝐭𝐡𝐞 𝐏𝐫𝐨𝐠𝐫𝐚𝐦:
The CHARGE Syndrome Foundation Family Assistance Program (FAP) was created in the Spring of 2020 in response to increased needs of families related to the COVID-19 pandemic.  Since its inception, the Foundation has supported over 100 families across the United States. The Foundation has expanded the program to include welcoming families of individuals who have been recently diagnosed with CHARGE syndrome with a special Foundation welcome package.

𝐑𝐞𝐪𝐮𝐢𝐫𝐞𝐦𝐞𝐧𝐭𝐬:  
Families with a newly diagnosed individual with CHARGE syndrome.
Must be registered with the Foundation.  
Family must reside in the U.S.

Who may request a welcome package be sent to a family of an individual with CHARGE?
●  Families of newly diagnosed individuals with CHARGE syndrome may request one for themselves
●  Extended family members, friends, educators or professionals who interact with a family of an individual with CHARGE
●  A CHARGE State Family Liaison
●  A member of the family’s state deaf-blind project
●  Foundation board, advisors, and employees

𝐀𝐩𝐩𝐫𝐨𝐯𝐚𝐥 𝐏𝐫𝐨𝐜𝐞𝐬𝐬:
*Please complete this online request form.  
*The Foundation will send you notification of the approval or denial within two weeks of your submission.  
*Once approved, welcome packages will be mailed within 5-7 business days to the recipient.  
*You must connect by phone or e-mail with the Director of Outreach or Foundation representative prior to the package being approved and mailed to you.
* Those previously registered with the Foundation are not eligible to receive the package.
